Alcohol Use in Rural and Urban India: Health Risks and Evidence

Alcohol use in rural and urban India is a public-health issue, but claims about who drinks more or which communities suffer most need a dated, clearly defined dataset. Health risks should be discussed without stereotyping people by income or where they live.

What health risks are established?

The World Health Organization identifies alcohol as a toxic, psychoactive substance that can produce dependence. Its fact sheet links alcohol use to diseases and injuries, including liver disease, some cancers and road-traffic harm. Risk depends on factors such as the amount consumed, the pattern of drinking and a person’s circumstances.

The US National Institute on Alcohol Abuse and Alcoholism explains that alcohol can affect the brain, heart, liver, pancreas and other body systems. Health risks can occur even at low levels of consumption; a branded drink is not inherently free of risk.

How to compare rural and urban consumption

India’s National Family Health Survey provides population survey evidence. The official NFHS-5 factsheet catalogue identifies the 2019–2021 survey period. These historical figures should not be presented as live 2026 measurements.

  • Check the age group and whether the measure covers men, women or the whole population.
  • Distinguish any reported consumption from frequency, quantity or alcohol dependence.
  • Use the same survey and definition for both locations.
  • Do not infer an individual’s behaviour from a regional average.

Social and household effects

WHO also describes harms extending beyond the drinker, including injuries and problems affecting families and communities. For a local assessment, relevant questions include spending, missed work, road safety, access to support and the availability of treatment.

It would be misleading to assume all rural drinking involves illicit products, or all urban drinking is driven by workplace stress. Such claims need specific evidence and should not replace an individual assessment.
